Home Choices offers a wide range of health care services that are provided in the comfort of our patient’s homes. Whether someone is recovering from a hospital stay or needs general assistance in day to day activities, Home Choices is able to provide the support they need to allow them to continue to live independently.
Through our wonderful staff, Home Choices provides a variety of home care services. We work directly with you, your family, and often a team of other health care professionals to make sure your loved ones are well taken care of.
Daily Living Activities
Includes cooking, cleaning, laundry, bathing, dressing, meal preps, medication, personal hygiene, and things that you need to do every day.
Medication Assistance
Helping keep your medicine organized and making sure it is taken on time, keeping you feeling your best.
In-Home Therapy
We can help continue your therapy in your home. We follow your PT, OT or SP by the recommendations directed by your care professionals.
Companionship
Companionships can be a ride around the lake or grabbing coffee or ice cream. Just having someone there even for a couple of hours.
Transporation
We provide transportation to and from doctors appointments, the pharmacy, family gathering and wherever your loved one needs to go.
Errands & Shopping
Picking up groceries and prescriptions, or taking something to the post office are just a few of the errands we routinely run for our clients.
Household Chores
Keeping your loved ones' house in order is a part of our job, helping keep their mind at ease.
Respite Care
We offer short-term, temporary care for your loved ones. If you need a break or some time to recover, respite care is perfect for you.
Coordination of Medical
